To be sure that your healthcare team knows your wishes and can honor them, St. Mary’s encourages you to complete the forms available on this page and to provide them to your primary care provider.

These directives help your loved ones and providers respect your wishes even if you are not in a position to tell them what you would like them to do. For example, if you are unconscious or severely injured, these directives give you a voice in your care.
