Youth Assertive Community Treatment (ACT) is a multidisciplinary program that delivers comprehensive, individualized community-based treatment to youth (ages 10 – 21) with complex mental health needs.
Referrals to the program are managed through the local Children’s Single Point of Access (C-SPOA). Program services are flexible, provided in the home or other community-based settings, and based on youth and family needs. Youth ACT staff collaborate with children and their families to identify strengths and challenges, establish short and long-term goals, and assist youth and their family in enhancing supports and developing skills that promote the young person’s growth, wellness, and an increased capacity to achieve their self-determined goals.
Program staff include:
Licensed mental health professionals
Licensed medication prescriber (psychiatrist or nurse practitioner)
Certified peer specialist
Clinical support worker
Program assistant
In-person services are available on weekdays from 8:00 AM to 4:00 PM. The program also maintains a 24/7 after-hours on-call service to provide support during crises.
